Regional Hospice is looking for FULL TIME Hospice RN Case Managers in our Community Hospice program covering Fairfield County, CT. We are looking for full time 40 hours, day shift and offer a generous benefits package including: Vision, Medical, Life, Dental, 401K Match, Vacation/Sick Time as well as the use of a company vehicle for work and personal use (after 3 months of employment).
The RN Case Manager is responsible for the coordination and management of all patient care and other services for patients and their families. The RN Case Manager is part of the interdisciplinary team and works with the patient’s primary care physician to provide quality pain and symptom management, as well as support to the patient and family.
Qualifications
- Registered Nurse with a current license in the State of Connecticut required.
- Bachelor of Science in Nursing degree required.
- Minimum of two (2) years' experience as a staff nurse in a hospital and at least one (1) year experience in hospice required.
- Wound care experience also needed.
- Advanced credentials in hospice care preferred (CHPN).
- Valid driver's license required, a clear driving history, and proof of auto insurance required.
- Excellent verbal, written and computer skills – ability to communicate effectively with patients, families, physicians, interdisciplinary team members and others.
- Fully Vaccinated for Covid-19
The RN Case Manager must possess good communication and organizational skills and be able to work in a variety of environments. The RN Case Manager must present a professional attitude and presentation at all times and be confident in hospice nursing skills, as independent judgement and decisions are often made.
